1 - Is this person having a mental health crisis?
If yes - proceed
2 - Does this person appear to be highly intoxicated?
If no, please proceed
3 - Do you believe this person can use the restroom and get in and out of bed without assistance?
If yes, please proceed
4 - Do they have any observable sores or injuries that need medical attention?
If no, please proceed
5 - Is this person ACTIVELY violent towards self or others?
If no, please call the number below to begin the referral process.
